I'm using Cuetools 2.1.6 to make my FLAC+CUE files, but that softwar eonly lets me create with 16/44 as input (even 16/48 doesn't work). It really has to be a clean PCM file. So I was hoping there might be other software that let's you use 24 bit files.

You could try fre:ac - free audio converter. Just drop your files in and select "Create cue sheet" and "Encode to a single file". It should transfer metadata, too.

EDIT: Ok, just tried fre:ac and it does work but you need to set General settings > DSP > Sample Format Converter to 24 bit to preserve the bit depth.

You can also invest in some commercial music converters with a multitude of features: dbPoweramp, xrecode, etc.
A free tool (probably sitting at the top of free digital music files manipulation) is Foobar2000... The use of it requires some reading but there's plenty of info available at: http://www.foobar2000.org/
